To get Rank S+ , the highest in the game, you can only do it from a game started from scratch ; not from playing New Game Plus. In other words: Capcom doesn’t want to make it possible to get S+ by “cheating” using tricks like extra weapons like the rocket launcher with infinite ammo.

In short, if we want to get the highest rank possible in Resident Evil 4 remake (S+), we have to start New Game from scratch and complete the game very quickly , which will force us to be very clear in advance what to expect and what to do at all times. For this reason, we do not recommend trying to get S+ until at least having completed the game a couple of times on different difficulty levels.
