These are the requirements that we must meet to get Rank A, Rank S and Rank S + in Resident Evil 4 remake :

To get Rank S+ , the highest in the game, you can only do it from a game started from scratch ; not from playing New Game Plus. In other words: Capcom doesn’t want to make it possible to get S+ by “cheating” using tricks like extra weapons like the rocket launcher with infinite ammo.
These are all the rewards we will get for completing the game in different difficulty modes and fulfilling different requirements:
- Wool Hat, Goggles (Square), and Iron Helmet : Complete the game on Standard Mode with Rank A.
- Deer Antlers : Complete the game on Standard Mode with an S rank.
- Goggles (Round), Pilot Helmet, and Surgical Mask : Complete the game on Hardcore Mode.
- Armor (Ashley’s Suit) : Complete the game on Hardcore Mode with Rank A.
- Chicken Hat (Leon) : Complete the game on Hardcore Mode with a S+ Rank.
- Gas Mask : Complete the game on Professional Mode.
- Hand Cannon – Complete the game on Professional Mode without using any extra weapons. Cannot be done in New Game Plus; must be done in a New Game.
- Chicago Sweeper : Complete the game on Professional Mode with an A Rank.
- Cat Ears : Complete the game on Professional Mode with an S+ rank.
In short, if we want to get the highest rank possible in Resident Evil 4 remake (S+), we have to start New Game from scratch and complete the game very quickly , which will force us to be very clear in advance what to expect and what to do at all times. For this reason, we do not recommend trying to get S+ until at least having completed the game a couple of times on different difficulty levels.
