LiveWell Partners acquires Michigan Community VNA Home Health and Hospice
LiveWell Partners acquired Michigan Community VNA Home Health and Hospice, expanding LiveWell’s home-based care footprint in Michigan and marking the company’s third acquisition in the state.
“Michigan Community VNA has a long history of serving patients and families with compassion and excellence, and we’re honored they’ve chosen to join us,” Jason Growe, LiveWell’s founder and chief development officer, wrote in a LinkedIn post. “Just as importantly, we’re committed to preserving the culture and community relationships they’ve built over the years while supporting their continued growth.”
Founded in 2023, LiveWell Partners is a St. Louis-based home health and hospice platform backed by private equity firm Encore Management Group. LiveWell operates in Illinois, Kansas, Michigan, Missouri and Ohio.
Michigan Community VNA offers Detroit-based skilled home health, hospice services, therapies and palliative care.
RN Enterprises buys Helping Hands Home Healthcare
RN Enterprises acquired Helping Hands Home Healthcare, to expanding its home healthcare offerings.
Jacksonville, Florida-based RN Enterprises provides home infusion nursing, clinical staffing and patient support services in 40 states, maintaining over 16 office locations and a network of more than 550 registered nurses.
Helping Hands Home Healthcare, also based in Florida, provides personal care services for older adults.
“This acquisition strengthens our ability to expand home-based healthcare while investing in our clinicians, supporting our partners, and improving patient outcomes,” RN Enterprises founder and owner Rodney G.B. Clements said in a statement.
The deal strengthens RN Enterprises’ ability to provide a broader continuum of care, according to the company. Helping Hands’ patient care will continue uninterrupted, and the entire Helping Hands team will remain in place, according to RN Enterprises.
Chapters Health System expands through Housecall Providers deal
Home-based care provider Chapters Health System has finalized its definitive agreement to purchase Portland, Oregon-based Housecalls Providers.
Temple Terrace, Florida-based Chapters Health System operates a portfolio of 30 companies and programs inlcuding the Program of All-Inclusive Care for the Elderly (PACE), Medicare Advantage plans and a nonprofit hospice network.
Housecalls Providers is a nonprofit organization offering team-based, in-home primary care and hospice services, as well as community-based palliative care services.
The move expands Chapters Health System throughout Oregon andbolsters Chapter’s value-based services through Housecall’s in-home primary care program, palliative care service line and hospice services.
“Together, we will be able to shape not only the future of Chapters Health, but the direction of chronic illness care as we continue to innovate new ways to improve the patient and caregiver experience,” Chapters Health System President and CEO Andrew Molosky said in a release.
Interim Healthcare offices in Texas and New Mexico rebrand to ExcelaCare
A franchisee of home health and hospice provider Interim Healthcare has rebranded to ExcelaCare.
The newly-minted ExcelaCare operates nine locations in Texas and at least three branches in New Mexico.
“While our logo and colors are changing, our ownership, management and the local teams you have come to know and trust are not,” the company said in a statement. “The decision to update our name to ExcelaCare was made with excellent care at the forefront, reflecting the same high standards our community has come to expect from us.”
Sunrise, Florida-headquartered Interim HealthCare provides home health and hospice services, alongside other offerings, from over 300 care centers across the U.S.
Patients currently receiving care from ExcelaCare will continue receiving care without disruption, according to the company.
Apex Infusion partners with SkyKnight Capital
Signal Hill, California-based infusion provider Apex Infusion Partners penned a definitive agreement to partner with private equity firm SkyKnight Capital to help grow Apex’s growth phase.
Founded in 2006, Apex operates approximately 40 ambulatory infusion locations. SkyKnight will help Apex expand into new markets, build strategic partnerships with prominent health systems and payers and improve patient access through technology-enabled initiatives, according to the company .
“With SkyKnight and FFL, we are well positioned to accelerate our expansion and extend Apex’s model of care to more patients nationwide,” Apex Executive Chairman and Co-founder Rick Smith said in a statement.
San Francisco-based middle-market private equity firm FFL Partners will maintain its position as a minority investor in Apex.
“We are excited to … continue building Apex into the alternate site infusion partner of choice for patients, providers and payers,” Don Wu, managing director at SkyKnight Capital, said in a statement. “Our investment approach is guided by a firm belief that industry leaders are built by exceptional management teams delivering uncommon value to their customers, employees and society.”
Prochant signs strategic growth partnership with Longshore Capital Partners
Prochant, a revenue cycle management platform for home-based care providers, plans to fuel its next growth phase through a strategic growth partnership with private equity firm Longshore Capital Partners.
Charlotte, North Carolina-based Prochant offers its revenue cycle management services to home medical equipment, durable medical equipment, infusion and specialty pharmacy organizations.
Through the partnership, Prochant intends to bolster client experiences and outcomes, scale and build out its talent recruitment and retention endeavors through the strategic growth partnership, according to the company. The company also plans to augment technological advancements, including AI and automation capabilities.
“This partnership gives us additional resources to build on what is already working,” said Prochant CEO Joey Graham in a statement. “We will continue listening closely to our clients, investing in our employees, and developing solutions that address the distinct revenue cycle challenges facing home-based care providers.”
